Zusammenfassung
Das Fachkrankenhaus Nordfriesland (FKH-NF) in Bredstedt/Schleswig-Holstein ist eine spezialisierte Einrichtung für umweltmedizinische Erkrankungen, Suchtkrankheiten sowie psychiatrische und psychosomatische Krankheitsbilder. Umweltmedizinische Patienten (UM-Patienten) mit meist chronischen Krankheitsbildern werden am FKH-NF stationär und ambulant betreut.
Dem therapeutischen Konzept des Fachkrankenhauses liegt ein multifaktorielles "bio-psycho-soziales" Krankheitsmodell zugrunde. Das Konzept ist dementsprechend mehrdimensional und besteht in erster Linie aus Schulungsmaßnahmen zur Verbesserung des Umgangs mit der Erkrankung (=Coping), aus Beratungen und Empfehlungen zur Expositionsminderung, zu Umstellungen bei der Ernährung und anderen Lebensstilfaktoren und zur Supplementierung von Nährstoffen sowie aus ergänzenden psycho- oder verhaltenstherapeutischen Maßnahmen.
In einer Verlaufsstudie mit 105 Patienten konnte gezeigt werden, dass vor allem Expositions-verminderung, Schulungen zum Coping und -in Einzelfällen- ergänzende psycho- oder verhaltenstherapeutische Maßnahmen positive Effekte hatten. Umweltmedizinische Patienten mit der Diagnose MCS (Multiple Chemical Sensitivity) profitierten am deutlichsten von der Therapie. Bei Patienten mit den höchsten Symptomwerten wurden die deutlichsten Verbesserungen erzielt. Weitere Prädiktoren für gesundheitliche Verbesserungen waren ein stimmiges Krankheitsmodell des Patienten, geringe Komorbidität und Mehrfachkontakte. Dies spiegelt vermutlich wider, dass die vielfältigen Beratungs- und Schulungsziele nur bei mehreren Besuchen erfolgreich vermittelt werden können. Aus diesem Grund kann das FKH-NF nun längere umweltmedizinische Rehabilitationsbehandlungen anbieten, die als besonders Erfolg versprechend einzustufen sind.

Abstract

Treatment options in Environmental Medicine and Multiple Chemical Sensitivity (MCS)
The Fachkrankenhaus Nordfriesland (FKH-NF) is a specialised clinic for environmental medicine, addiction and psychosomatic disorders in Northern Germany. Patients with mostly chronically environmental illness (EI) are treated in inpatient and outpatient facilities.
The therapeutically concept of the clinic is based on a "bio-psycho-social" model of disease. The concept is therefore multidimensional and consists in the first step of training measures to improve coping; counselling and recommendations for the reduction of exposure, changes in diet and lifestyle habits and supplementation of nutrients; as well as complementary psycho-therapeutic and behavioural measures.
The results of a follow-up study on 105 patients showed that minimising exposure to relevant pollutants, training for coping and -in single cases- complementary psycho- or behavioural therapy had the best positive effect values. Patients with high initial symptom scores profited most of the therapy Further predictors of improvement were a low contribution of comorbidity, a coherent belief system concerning the illness, a diagnosis of chemical sensitivity and repeated visits to the clinic. This presumably reflects the fact that the many different aims of training and counselling can only be adequately communicated through multiple contacts. On the basis of these results the clinic now offers opportunities for longer stays for the rehabilita-tion (REHA) of severely afflicted patients.
